ln order to maintain the highest quality of dental treatment and care for our patients, all financial obligations must be taken care of as they become due.

Your insurance policy is an agreement between you and the insurance company. We will flle all the necessary insurance forms for your treatment within a reasonable time. However, if your insurance company should deny payment for any reason, the outstanding balance is to be paid directly by you. Please remember, insurance is considered a method of REIMBURSING THE PATIENT for fees paid to the doctor and is not a substitute for payment.

In our effort to provide timely, quality care at a reasonable fee, it is extremely important to maintain an efficient appointment schedule. If your appointment time requires rescheduling, a minimum of twenty-four (24) hours notification is necessary to our office. It is with regret that if cancellation within twenty-four hours or non-utilization of your appointment occurs, you will responsible for a cancellation fee.

Personal checks are accepted as a method of payment at our office. Should your check not clear with our bank, a return check fee of $25.OO will be assessed onto your account.

Our office offers financial arrangements that can assist you in meeting your financial needs. Should your account default with our office, any and all collection and attorney any and all will be the sole responsibility of the patient in addition to the outstanding balance.
